On August 6, 2026, HIV service providers from around Alameda and Contra Costa counties gathered for the August 2026 Regional Case Managers meeting to learn about the role of Public Health Investigators in supporting linkage for clients who are out of care and explore Alameda County Healthcare for the Homeless’ Medically Supported Housing program.
These meetings are a place for those working directly with clients to come together, network, get access to important updates and problem-solve the challenges they face in their work.
Objectives for the June 2026 RCM meeting:
- Learn about the role of Public Health Investigators (PHI) within Alameda County and develop a process to coordinate with PHIs to support linkage for clients who are out-of-care.
- Learn about the Medically Supported Housing program of Alameda County’s Healthcare for the Homeless which offers permanent and interim housing for residents who need more medical support than offered in traditional permanent supportive housing.
- Develop relationships with colleagues from other agencies providing case management services.
