Bay Area Community Health (BACH) is a Federally Qualified Health Center with multiple clinic locations in Fremont, San Jose, and Gilroy as well as community-based mobile clinic deployments throughout Southern Alameda and Santa Clara Counties serving the whole family.
Our HIV Care Clinic and Ryan White Program is based at our Mowry 2, Suite F location in Fremont providing HIV primary care services for over 30 years. We also have gender-affirming care services for our trans and gender-diverse community members as well as access to oral or long-acting injectable Pre-Exposure Prophylaxis (PrEP) and non-occupational Post-Exposure Prophylaxis (nPEP).

Bay Area Community Health also provides HIV/AIDS services funded by the Ryan White program such as: Ambulatory Medical Care, Behavioral Health, Case Management, Nutrition Services, Psych Social Support (support groups), Early Intervention Services. Hormone replacement therapy. PrEP and PEP assistance. Narcan, Syringe Access, Fentanyl/Xylazine testing strips.
We have a variety of support groups for PLWHA such as English and Spanish psychosocial, nutrition and substance abuse support groups
