Access Information

Access during COVID outbreak:
-Most visits are done by phone; very limited in-person visits available based on provider assessment of necessity. Please call to schedule before coming in. Phone support available for the following services:
-outpatient ambulatory, vision, medical case management, mental health, substance abuse, medical nutrition therapy, early intervention services, psychosocial support services: call 510-770-8040
-dental services: call 510-252-6803 for urgent or emergency services

People living with HIV who are uninsured, on Medi-Cal, Alameda Alliance, Blue Cross, Medi-Care, Health Pac

Core services:

Assistance with insurance benefits counseling (Medi-Cal/Medicare, Health Pac, Covered CA); ADAP/OAHIP enrollment; Case Management Services; Transgender Program/Transvision services; Assistance with long-term/ short term housing (HOPWA); Transportation assistance to and from medical visits; Emergency Financial Assistance; PrEP and PEP available; Language line on-site interpretation

Additional services on-site:

We have a variety of support groups for PLWHA such as English and Spanish psychosocial, nutrition and substance abuse support groups

Required information for referrals:
Name, Release of information from previous provider, past labs, medication list, documentation of HIV + test result. Newly diagnosed- confirmation test results.
