Access Information
Access during COVID outbreak:
-Project Open Hand’s Grocery Center is open from 10:00am-2:00pm Monday, Tuesday, Thursday, and Friday. Closed Wednesday.
Admin hours are Monday-Friday 8:00am-3:30pm
-Clients will receive prepackaged grocery bags or prepackaged medically tailored meals
-Nutrition counseling by a Registered Dietitian—telephonic during shelter in place
- We welcome:
People with chronic health conditions, including: HIV/AIDS, Hepatitis C, Congestive Heart Failure, Type 1 and Type 2 diabetes with an HbA1c of 8.0% or above, Recent major surgery, within 30 days of discharge
- Same-day access and availability:
Same day access with completed application
- Core services:
Medically tailored frozen meals and/or fresh groceries. Clients can shop once a week. Nutrition counseling by a Registered Dietitian.
