Regional Case Managers meeting – February 2026

 

 

   

 

Calling all Alameda/Contra Costa case managers, outreach workers, peer advocates, patient navigators and others who support people living with HIV OR people accessing prevention!

Come network with others in this field, stay up to date on program and medical information and access professional development opportunities.

Date: Thursday, February 12, 2026

Intention: These meetings are a place for those working directly with clients to come together, network, get access to important updates and problem-solve the challenges they face in their work.

Objectives: 

  • Identify Project Open Hand’s regional efforts to improve health outcomes. Become familiar with eligibility requirements and referral processes for POH’s programs, including medically tailored meals and groceries.
  • Describe Rubicon Programs’ services provided under the four pillars of Assets, Income, Wellness, and Connections and identify ways to refer clients to Rubicon’s services. 
  • Identify strategies to help ensure individuals in crisis connect with supportive, caring professionals and promote proactive help-seeking behaviors within the community. Become familiar with CSS crisis services, including both the call and text lines.
