Regional Case Managers meeting – August 2025

Calling all Alameda/Contra Costa case managers, outreach workers, peer advocates, patient navigators and others who support people living with HIV OR people accessing prevention!

Come network with others in this field, stay up to date on program and medical information and access professional development opportunities.

Intention: These meetings are a place for those working directly with clients to come together, network, get access to important updates and problem-solve the challenges they face in their work.

Objectives:

• Learn about the substance use disorder (SUD), behavioral health, and medication-assisted treatment (MAT) services available through Alameda Health System’s Road to Recovery program and the Bridge Clinic.
• Identify appropriate referral pathways for clients with substance use disorders (SUD) to access behavioral health and MAT services within Alameda Health System.
• Discuss the role of harm reduction strategies in supporting engagement and retention in SUD treatment at the Road to Recovery program and Bridge Clinic.
