Regional Case Managers meeting – April 4th, 2024
Calling all Alameda/Contra Costa case managers, outreach workers, peer advocates, patient navigators and others who support people living with HIV OR people accessing prevention!
Come network with others in this field, stay up to date on program and medical information and access professional development opportunities.

Location: Block Community Hub Event Space (1955 Broadway, Oakland, CA 94612)
Intention: These meetings are a place for those working directly with clients to come together, network, get access to important updates and problem-solve the challenges they face in their work.

- Community resources: Medi-Cal waiver program and home/community-based health services
- Housing updates: Coordinated entry system, assessment & acuity scores, housing options for your homeless clients
- Case management fundamentals: Enhancing Case Management Standards: A Focus on Client Relationships, Boundaries, Self-Disclosure, and Documentation
- Community announcements
- Breakout group activity
- Close out and next meeting information
- Grab & go lunch available
Please note: We are continuing to monitor developments around COVID-19 and are recommending the following:
